What Is IT Band Syndrome?
The IT band is a thick band of connective tissue running from the hip down the outer thigh to the lateral knee. At approximately 30 degrees of knee flexion (the position the knee is in during the mid-stance phase of running), the IT band passes over a prominent bony ridge on the outer femur, creating a friction zone that can become inflamed with repetitive motion.
Signs and Symptoms
- Sharp or burning pain on the outer knee, typically starting at a consistent point during a run
- Pain that is absent at rest and with short distances, then reliably appears with sustained activity
- Tenderness at the lateral femoral epicondyle
- Pain when walking downstairs or with sustained knee bending
- Tightness of the outer thigh and hip
Common Causes and Risk Factors
- Sudden increases in training volume or intensity
- Running on cambered surfaces or consistent road-side running
- Hip abductor weakness allowing excessive hip drop during the running stride
- Tight hip flexors and IT band
- Leg length discrepancy or excessive foot pronation
How It Is Diagnosed
Diagnosis is primarily clinical based on the characteristic symptom pattern and tenderness at the lateral femoral epicondyle. Ultrasound can confirm IT band thickening and fluid accumulation in the friction zone.
