Intradiscal Injection
What Are Intradiscal Injections?
Intradiscal injections involve precise placement of therapeutic solutions directly into spinal discs to address disc tears, degenerative disc disease, and discogenic pain. This advanced procedure is performed under fluoroscopic (X-ray) guidance to ensure accurate needle placement in these small, challenging-to-access structures.
At Palm Beach Regenerative, Dr. Ghattas uses intradiscal injections selectively as part of comprehensive spine care, following the Regenexx protocols for disc treatment.
Important: The majority of our spine procedures use platelets and target facet joints, ligaments, and nerves rather than the disc itself, as disc injection is more complex and reserved for specific situations.
How Intradiscal Injections Work
Understanding Discogenic Pain:
Spinal discs act as shock absorbers between vertebrae. When discs develop tears in their outer layer (annulus), nerves grow into these tears causing pain that worsens with sitting, bending, and lifting.
The Procedure:
- Pre-Procedure Preparation
Detailed MRI review to identify disc pathology and confirm disc as pain source. - Fluoroscopic Guidance
Under continuous X-ray guidance, Dr. Ghattas:- Advances a needle through back muscles to the disc
- Confirms position within disc using contrast dye
- Injects therapeutic solution (typically platelet preparations or bone marrow concentrate)
- May treat multiple disc levels if indicated
- What We Inject:
- Platelet-rich plasma or platelet lysate (most common)
- Bone marrow concentrate (select cases)
- Combinations based on individual assessment
These preparations aim to support your body’s healing response in the disc, reduce inflammation, promote healing of annular tears, and improve disc health.
Total Time: Plan for 2-3 hours for complete visit
Conditions Treated with Intradiscal Injections
Disc Conditions:
- Annular tears (disc outer layer tears)
- Degenerative disc disease
- Discogenic pain (pain originating from disc)
- Early disc degeneration
- Post-discectomy pain syndrome (select cases)
Symptoms We Address:
- Axial back or neck pain (midline, disc-related)
- Pain worsening with sitting or forward bending
- Pain not adequately explained by other structures
- Chronic disc-related discomfort
What Makes Intradiscal Injections Different?
Direct Disc Treatment:
Unlike facet injections or epidurals, intradiscal injections deliver treatment directly into the disc structure.
Fluoroscopic Precision:
Continuous X-ray guidance ensures accurate needle placement in these small, deep structures.
Part of Comprehensive Spine Care:
IMPORTANT: At Palm Beach Regenerative, we take a comprehensive approach to spine pain:
- Most spine procedures target facet joints, ligaments, and nerves
- We use primarily platelet preparations (not just stem cells)
- Disc injection is reserved for specific, carefully selected cases
- We treat the whole spinal unit, not just the disc
The Regenexx Approach:
- Thorough evaluation to determine if disc is pain source
- Assessment of surrounding structures (facets, ligaments, nerves)
- Often treating multiple structures in one session
- Disc injection only when clearly indicated

Intraosseous BMC
What are Intraosseous Injections?
Intraosseous (IO) injections represent an advanced Regenexx technique that addresses pain and pathology beneath the cartilage surface – directly within the bone. Many patients with joint pain have bone marrow lesions (BMLs) visible on MRI that contribute significantly to symptoms yet are inaccessible to traditional treatments.
At Palm Beach Regenerative, Dr. Ghattas uses precision fluoroscopic guidance to deliver therapeutic biologics directly into these bone lesions, targeting an often-overlooked source of joint pain.
Bone is highly innervated, so BMLs cause significant pain that standard intra-articular injections cannot adequately address.
How IO Injections Work
Understanding Bone Marrow Lesions:
MRI imaging may reveal areas of bone marrow swelling and inflammation (bone marrow lesions or BMLs) in joints affected by:
- Osteoarthritis
- Stress reactions
- Avascular necrosis (early stages)
- Post-traumatic changes
- Overload and repetitive stress
The Procedure:
- Fluoroscopic Guidance
Under continuous X-ray guidance, Dr. Ghattas:- Advances a specialized needle through skin to bone surface
- Penetrates cortical (outer) bone layer
- Enters bone marrow cavity at lesion site
- Confirms position with contrast dye
- Injects therapeutic biologic preparation
- May treat multiple lesion sites if needed
- What We Inject:
- Bone marrow concentrate (contains cells that may support bone healing)
- Platelet preparations
- Combinations based on individual assessment
These preparations aim to support your body’s healing response in bone, reduce inflammation within bone marrow, promote resolution of bone marrow lesions, and improve bone health.
Total Time: Plan for 2-2.5 hours
Conditions Treated with IO Injections
Bone Marrow Lesions:
- BMLs associated with knee osteoarthritis
- BMLs in hip, ankle, or other joints
- Post-traumatic bone bruising and swelling
- Stress reactions in bone
Early Arthritis with Bone Involvement:
- Arthritis with significant bone pain component
- Subchondral bone plate changes
- Bone overload from malalignment
Other Bone Conditions:
- Early avascular necrosis (limited cases)
- Bone cysts or lesions (select cases)
- Failed cartilage-only treatments
What Makes IO Injections Different?
Direct Bone Treatment:
Unlike intra-articular injections (into joint space), IO injections treat the bone directly where pain originates.
Targets Hidden Pain Source:
Many patients have significant bone pathology missed by cartilage-focused treatments.
Advanced Regenexx Technique:
Requires specialized training, equipment, and fluoroscopic skills. Not widely available.
Comprehensive Joint Treatment:
Often combined with intra-articular injection for complete joint treatment:
- IO injection for bone marrow lesions
- Intra-articular for cartilage and joint lining
- Addresses multiple pain sources in one session
What to Expect After IO Injections
Immediate (Day 0-3):
- Post-procedure bone pain expected (drilling into bone causes soreness)
- May be more uncomfortable than standard injections
- Ice, rest, and pain management as directed
- Avoid weight-bearing on affected limb as directed
- Avoid anti-inflammatory medications
Early Phase (Week 1-4):
- Gradual reduction in post-procedure soreness
- Protected weight-bearing continues
- Begin gentle range of motion
- Some patients notice early improvement
Healing Phase (Week 4-12):
- Bone healing and remodeling occurs
- BML resolution may be visible on follow-up MRI
- Progressive improvement in pain and function
- Gradual return to activities
Maturation Phase (3-6 months):
- Continued bone remodeling
- Maximum benefit often at 6 months
- Follow-up MRI may show BML improvement
- Long-term activity modifications discussed
Activity Restrictions:
- Protected weight-bearing: 2-6 weeks (varies by joint and lesion)
- Avoid high-impact activities: 8-12 weeks
- Gradual return to sport/activities
- Joint protection and alignment correction important
Success Rates and Outcomes
For appropriately selected patients with bone marrow lesions, many report:
- Significant reduction in deep, aching bone pain
- Improved function and weight-bearing tolerance
- Enhanced response to overall joint treatment
- MRI evidence of BML improvement (in many cases)
Important Factors:
- Size and number of BMLs
- Underlying cause of bone pathology
- Overall joint health
- Concurrent treatments
- Activity modifications
Realistic Expectations:
- Bone healing takes time (months)
- Some BMLs respond better than others
- Not all bone pain is from BMLs
- May need to address underlying biomechanical issues
- Some patients ultimately need surgery for advanced arthritis
Is IO Injection Right for You?
May be appropriate when:
- MRI shows bone marrow lesions contributing to pain
- You have significant bone pain component (deep, aching)
- Cartilage-focused treatments haven’t fully addressed symptoms
- You want comprehensive joint treatment
- Early arthritis with bone involvement
Often combined with:
- Intra-articular orthobiologic injection
- Realignment procedures if malalignment present
- Physical therapy for joint protection
- Activity modification and biomechanical correction
May NOT be best choice when:
- No BMLs identified on MRI
- Severe, end-stage arthritis (surgery likely better)
- Active infection in joint or bone

Platelet Lysate
What is Platelet Lysate (PL) Therapy?
Platelet Lysate (PL) represents a specialized preparation of platelets designed to deliver growth factors in a form particularly well-suited for certain conditions, especially those involving nerves and inflammatory processes.
Through a controlled freeze-thaw process that ruptures platelet membranes, growth factors are released into solution, creating a preparation with unique anti-inflammatory properties and nerve-friendly characteristics.
At Palm Beach Regenerative, Dr. Ghattas often uses PL for nerve-related conditions and as part of comprehensive treatment protocols, frequently combining it with nerve hydrodissection techniques.
How PL Treatment Works
- Blood Draw
A small blood sample is drawn from your arm (30-60cc). - Laboratory Processing
Your blood undergoes platelet concentration through centrifugation, followed by a controlled freeze-thaw cycle to create lysate. This releases growth factors into solution. - Image-Guided Injection
Using ultrasound or fluoroscopic guidance, Dr. Ghattas precisely injects PL around nerves or into joints/tissues as appropriate.
Total Time: Approximately 1.5-2 hours.
Conditions Treated With Platelet Lysate
PL therapy may help patients with:
Nerve Conditions:

- Added to BMC or SCP for inflammatory conditions
- Spinal facet joint injections
- Around nerve roots for radicular pain
What Makes PL Different?
Pre-Released Growth Factors:
Growth factors are already in solution, potentially offering immediate availability to tissues.
Nerve-Specific Applications:
The composition of PL may be particularly beneficial for nerve tissue, which can be sensitive to intact platelets.
Anti-Inflammatory Profile:
May offer enhanced anti-inflammatory effects compared to standard platelet preparations.
Versatility:
Can be used alone or combined with other interventional orthobiologics for synergistic effects.
What to Expect After PL Treatment
Immediate (Day 0-3):
- Minimal post-procedure soreness (often less than SCP/BMC)
- May experience temporary numbness if local anesthetic used
- Resume normal activities as tolerated
Early Response (Week 1-4):
- Nerve conditions may show early improvement
- Reduction in nerve-related symptoms (tingling, numbness, pain)
- Anti-inflammatory effects become apparent
Continued Improvement (Week 4-12):
- Ongoing reduction in symptoms
- May require repeat treatments for optimal results
- Often combined with other therapies
Success Rates and Outcomes

Microfragmented Adipose Tissue
What is MFAT Therapy?
Microfragmented Adipose Tissue (MFAT) therapy uses regenerative cells naturally found in your body’s fat tissue to support healing in joints, tendons, and other musculoskeletal structures.
Through a minimally invasive procedure, fat tissue is harvested, processed to preserve cell viability, and precisely injected where your body needs healing support. At Palm Beach Regenerative, Dr. Ghattas offers MFAT as an alternative or complement to bone marrow concentrate for carefully selected patients.
Adipose tissue contains a rich population of regenerative cells, growth factors, and supporting structures that work together to support tissue healing.
How MFAT Treatment Works
- Fat Harvest
A small amount of fat is harvested from your abdomen or flank using local anesthesia and a small cannula. The harvest is gentle and well-tolerated with minimal discomfort. - Laboratory Processing
Fat tissue is processed in a sterile closed system using gentle microfragmentation. This minimal processing preserves the natural structure and viability of cells. - Image-Guided Injection
Using ultrasound or fluoroscopic guidance, Dr. Ghattas precisely injects MFAT to target structures in joints, tendons, or other tissues.
Total Time: Plan for 2.5-3 hours for complete visit.
Conditions Treated With MFAT
MFAT therapy may help patients with:
Joint Conditions:
- Knee osteoarthritis (mild-moderate)
- Hip osteoarthritis (select candidates)
- Shoulder arthritis
- Small joint arthritis
Tendon Conditions:
- Rotator cuff tears (partial)
- Achilles tendinopathy
- Chronic tendon degeneration
Other Applications:
- Ligament injuries
- Cartilage damage
- Chronic inflammation
- Conditions requiring substantial cell populations
What Makes MFAT Different?
Alternative Cell Source:
For patients where bone marrow harvest is not ideal, MFAT provides another option using fat tissue.
Abundant Cell Population:
Fat tissue contains high concentrations of regenerative cells in a small volume.
Minimally Invasive Harvest:
Fat harvest via small cannula is generally well-tolerated with minimal discomfort.
Preserved Natural Structure:
Minimal processing maintains cells in their natural environment, potentially preserving function.
What to Expect After MFAT Treatment
Immediate (Day 0-3):
- Mild soreness at harvest site (similar to minor bruise)
- Some swelling or discomfort at injection site
- Ice and compression as directed
- Return to light activities day 1-2
Early Phase (Week 1-4):
- Harvest site heals quickly (usually within 1 week)
- Injection site inflammation part of healing response
- Begin gentle exercises as directed
Regeneration Phase (Week 4-12):
- Gradual improvement in symptoms
- Tissue healing and remodeling continues
- May be combined with physical therapy or MLS Laser
Maturation Phase (3-6 months):

Bone Marrow Concentrate
What Is Bone Marrow Concentrate (BMC)?
Bone marrow concentrate is one of the most advanced interventional orthobiologics available, using concentrated healing cells from your own bone marrow to support tissue repair and reduce pain.
Your bone marrow contains mesenchymal stem cells (MSCs), growth factors, and other healing components that may help support healing in damaged cartilage, ligaments, tendons, and discs.
Unlike synthetic treatments or cortisone injections that provide temporary relief, BMC treatments support your body’s natural healing response for potentially long-lasting results.
How BMC Treatment Works
The procedure is performed in a single office visit:
- Bone Marrow Harvest
A small amount of bone marrow is harvested from your pelvis using local anesthesia. Most patients report minimal discomfort—often less than a dental procedure. - Advanced Processing
Your bone marrow is processed in our certified lab using proprietary Regenexx protocols. This concentrates the healing cells and growth factors while removing components that don’t contribute to healing. - Image-Guided Injection
Using real-time ultrasound or fluoroscopic guidance, Dr. Ghattas precisely injects the concentrated cells into damaged tissue. This precision ensures optimal treatment delivery by placing cells exactly where needed.
Conditions Treated With BMC
BMC is typically used for moderate to severe orthopedic conditions requiring maximum regenerative potential:
- Moderate to severe knee, hip, or shoulder arthritis
- Large rotator cuff tears (partial thickness)
- Meniscus tears
- ACL or other ligament injuries
- Hip labral tears
- Disc tears and degenerative disc disease
- Cartilage defects
- Severe tendon tears (Achilles, patellar)
BMC vs. Other Regenerative Treatments
BMC contains a more diverse population of healing cells compared to PRP or other platelet treatments:
- Mesenchymal stem cells with regenerative potential
- Higher concentrations of growth factors
- Anti-inflammatory cytokines
- Structural proteins supporting tissue scaffold
This makes BMC particularly effective for conditions with significant tissue damage or degeneration.
What to Expect After BMC Treatment
Immediate (Days 1–3)
- Some soreness at harvest and injection sites
- Rest from aggravating activities
- Ice and elevation as needed
- Most patients resume desk work within 1–2 days
Weeks 1–6
- Gradual reduction in pain
- Begin gentle, progressive activity
- Follow rehabilitation protocol
- First follow-up at 6 weeks
Months 2–4
- Continued functional improvement
- Return to most activities
- May notice further gains as healing progresses
Months 4–12
- Ongoing tissue maturation
- Maximum improvement typically seen by 6–12 months
- Long-term durability of results
Success Rates and Outcomes
Based on Regenexx registry data from thousands of BMC procedures, many patients report:
- Significant improvement in pain and function
- Avoided or delayed surgery they were previously considering
- Meaningful pain reduction
- Improved functional capacity
Results vary significantly by individual. Dr. Ghattas will discuss realistic expectations based on your specific condition.
Is BMC Right for You?
BMC may be appropriate if you:
- Have moderate to severe arthritis or large tears
- Want to explore alternatives to surgery
- Have not achieved adequate relief from conservative treatments (PT, medications)
- Are looking for long-term solution, not temporary relief
- Are willing to participate in structured follow-up
BMC may not be appropriate if you:
- Have very mild conditions (SCP may be sufficient)
- Have end-stage bone-on-bone arthritis (surgery may be more appropriate)
- Cannot participate in rehabilitation
- Need immediate symptom relief (BMC takes time to work)
Dr. Ghattas will review your imaging and discuss honestly whether BMC is your best option.
Why Choose Palm Beach Regenerative for BMC?

- Superbills for possible HSA/FSA reimbursement or out-of-network submission
- CareCredit financing options available
Total costs vary based on number of areas treated and complexity of injections. Contact us for specific pricing. Schedule a consultation to discuss BMC treatment.
Required Disclaimer
Like all medical procedures, BMC therapy has potential risks and benefits. Results vary by individual based on condition severity, overall health, and healing response. Not all patients are candidates. Dr. Ghattas will discuss realistic expectations and all treatment options including when surgery may be more appropriate.
Frequently Asked Questions
Most patients tolerate BMC treatment very well. The bone marrow harvest is performed under local anesthesia, and many patients report discomfort similar to or less than a dental procedure. Mild soreness at the harvest and injection sites is common for a few days.
Some patients notice improvement within the first few weeks, but meaningful results typically develop over 2–3 months as tissue healing progresses. Maximum improvement is often seen between 6–12 months.
BMC contains a broader range of healing components, including mesenchymal stem cells, growth factors, and anti-inflammatory cytokines. It is generally used for more advanced or severe conditions, whereas PRP is often appropriate for milder injuries or earlier-stage degeneration.

Super Concentrated Platelets
What is Super Concentrated Platelet (SCP) Therapy?
Super Concentrated Platelet (SCP) therapy represents an advanced evolution of platelet-rich plasma (PRP) treatment. Through sophisticated processing techniques, SCP delivers significantly higher concentrations of growth factors than standard PRP preparations – often 5-10 times higher.
These concentrated platelets contain powerful growth factors that support your body’s natural healing response by signaling repair cells to increase their activity, reducing inflammation, and enhancing blood flow to damaged tissues.
At Palm Beach Regenerative, Dr. Ghattas uses precision image guidance to deliver SCP exactly where your body needs it most, targeting tendons, ligaments, and joints affected by injury or degeneration.
How SCP Treatment Works
The procedure is performed in a single office visit:
- Blood Draw
A small blood sample is drawn from your arm (30-60cc depending on treatment area) – similar to routine lab work. - Advanced Laboratory Processing
Your blood is processed in our certified in-office laboratory using advanced centrifugation techniques that concentrate platelets to levels 5-10 times higher than standard PRP. - Image-Guided Injection
Using real-time ultrasound guidance, Dr. Ghattas precisely injects the SCP into the exact target area – whether tendon, ligament, or joint.
Total Time: Plan for approximately 2 hours for your complete visit.
Conditions Treated With SCP
SCP therapy may help patients with:
Tendon Conditions:
- Tennis elbow (lateral epicondylitis)
- Golfer’s elbow (medial epicondylitis)
- Achilles tendinitis
- Patellar tendinitis (jumper’s knee)
- Rotator cuff tendinopathy
- Plantar fasciitis
Ligament Injuries:
- Partial ligament tears
- Chronic ligament laxity
- Joint instability
Mild to Moderate Arthritis:
- Knee osteoarthritis (mild-moderate)
- Hip osteoarthritis (carefully selected candidates)
- Shoulder arthritis
- Hand and wrist arthritis
Other Conditions:
- Muscle strains
- Chronic tendon degeneration
SCP vs. Other Treatments
Higher Growth Factor Concentration:
SCP preparations contain significantly more growth factors than standard PRP, potentially offering enhanced healing support for stubborn conditions.
Minimal Inflammatory Components:
Advanced processing reduces pro-inflammatory white blood cells while preserving beneficial healing factors.
Customized Preparation:
Dr. Ghattas adjusts the preparation protocol based on your specific condition and the tissue being treated.
Image-Guided Precision:
Every SCP injection is performed under real-time ultrasound guidance, ensuring accurate placement at the injury site.
What to Expect After SCP Treatment
Immediate (Day 0-3):
- Some soreness or mild swelling at injection site is normal
- May feel temporary increase in discomfort (this is part of healing response)
- Ice and rest as directed
- Avoid anti-inflammatory medications (they can interfere with healing)
Early Phase (Week 1-2):
- Gradual reduction in post-procedure soreness
- Begin gentle range of motion exercises as directed
- Some patients notice early improvement, others at 4-6 weeks
Regeneration Phase (Week 2-8):
- Body’s healing response continues
- Gradual improvement in pain and function
- May be combined with physical therapy or MLS Laser
Maturation Phase (Week 8-12+):
- Continued improvement in symptoms
- Tissue remodeling continues for 3-6 months
- Follow-up imaging may be performed
Activity Restrictions:
- Avoid high-impact activities: 4-6 weeks
- Gradual return to sport/activity as tolerated
- Specific restrictions based on condition and response
Success Rates and Outcomes
Based on Regenexx registry data and published literature, many patients with tendon and ligament conditions report:
- Meaningful reduction in pain levels
- Improved function and mobility
- Return to activities that were previously limited
- Reduced need for pain medications
Important: Results vary significantly based on severity and chronicity of condition, overall health, adherence to protocols, and individual biological response.
Is SCP Right for You?
SCP may be appropriate when:
- You have a tendon or ligament condition not responding to conservative care
- You have mild-moderate arthritis and want to explore non-surgical options
- You want to avoid or delay surgery
- You prefer using your own healing biology rather than cortisone injections
SCP may NOT be best choice when:
- You have severe, bone-on-bone arthritis (surgery may be more appropriate)
- You have a complete tendon or ligament rupture requiring repair
- You need immediate symptom relief (SCP takes weeks to months for effect)
Compared to Bone Marrow Concentrate (BMC):
- SCP is less invasive (only blood draw, no bone marrow harvest)
- BMC contains additional cell types that may benefit more severe conditions
- Dr. Ghattas will recommend which treatment best fits your needs
Why Choose Palm Beach Regenerative for SCP?
- Regenexx-certified advanced platelet protocols
- Image-guided precision with every injection
- On-site laboratory processing
- Fellowship-trained physician
- Systematic outcome tracking
- Can be combined with MLS Laser for enhanced results

Trigger Point Injections
What are Trigger Point Injections?
Trigger Point Injections (TPI) provide targeted treatment for painful muscle knots (trigger points) that cause localized pain and may refer pain to other areas of the body. These taut bands of muscle fiber can develop from injury, overuse, poor posture, or stress.
At Palm Beach Regenerative, Dr. Ghattas often combines TPI with interventional orthobiologics to address both muscle-based and structural sources of pain for comprehensive musculoskeletal care.
Trigger points are hyperirritable spots in skeletal muscle that feel like firm nodules, are tender to touch, and can limit range of motion while causing muscle weakness.
How TPI Works
- Trigger Point Identification
Dr. Ghattas performs physical examination to identify and mark trigger points causing pain. - Injection
A small needle is inserted directly into the trigger point. Solutions used may include:- Local anesthetic (lidocaine or bupivacaine)
- Saline solution (for mechanical disruption)
- Platelet preparations (for enhanced healing)
- Combination approaches for optimal results
- Release
The injection disrupts the trigger point, delivers therapeutic solution, reduces local inflammation, and allows muscle to relax and function properly.
Total Time: Approximately 30-45 minutes
Conditions Treated with TPI
Myofascial Pain Syndrome:
- Chronic muscle pain from trigger points
- Multiple trigger points causing widespread discomfort
- Pain that doesn’t respond to conservative therapy
Specific Muscle Groups:
- Trapezius (upper back/neck)
- Levator scapulae (neck/shoulder blade)
- Rhomboids (between shoulder blades)
- Quadratus lumborum (low back)
- Piriformis (hip/buttock)
- Any skeletal muscle with active trigger points
Associated Conditions:
- Tension headaches from neck trigger points
- TMJ-related muscle pain
- Fibromyalgia (as part of comprehensive treatment)
- Sports injuries with muscle involvement
- Post-injury muscle guarding and spasm
What Makes TPI Different?
Immediate Relief:
Many patients experience rapid improvement, often within minutes to hours of treatment.
Precise Targeting:
Direct injection into specific trigger points provides focused treatment unlike oral medications (systemic effects), general massage (less specific), or topical treatments (limited penetration).
Combination Potential:
Often combined with:
- Orthobiologic injections for joints or tendons
- Physical therapy for strengthening
- MLS Laser for inflammation
- Comprehensive approach for complex pain
Enhanced with Orthobiologics:
When appropriate, Dr. Ghattas may add platelet preparations to trigger point injections for enhanced tissue healing support.
What to Expect After TPI
Immediate (Day 0-1):
- Often notice immediate improvement from anesthetic
- May feel post-injection soreness (like after deep massage)
- Some bruising possible at injection sites
- Apply ice as directed
Early Response (Day 1-3):
- Post-procedure soreness resolves
- Trigger point pain significantly reduced
- Improved range of motion
- Enhanced function
Continued Improvement (Week 1-4):
- Sustained relief in many cases
- Muscle function continues to normalize
- May combine with physical therapy or stretching
- Some patients need repeat treatments
Long-Term:
- Lasting relief in responsive cases
- Address underlying causes (posture, ergonomics, stress)
- Occasional maintenance treatments may be needed
- Lifestyle modifications important for preventing recurrence
Success Rates and Outcomes
Many patients with trigger point pain report:
- Significant reduction in muscle pain
- Improved range of motion
- Enhanced ability to perform daily activities
- Reduced muscle tension and tightness
- Decreased frequency of headaches (if trigger points in neck)
Response Factors:
- Acuity vs. chronicity of trigger points
- Number and location of trigger points
- Underlying contributing factors
- Concurrent treatments (therapy, ergonomics, stress management)
Is TPI Right for You?
May be appropriate when:
- You have identified trigger points causing pain
- Conservative treatments (massage, therapy) provide only temporary relief
- You have myofascial pain syndrome
- Muscle pain limits your function or quality of life
- You want rapid symptom relief
Often combined with:
- Interventional orthobiologics for joints or tendons
- Physical therapy for muscle retraining
- MLS Laser therapy
- Ergonomic and posture modifications
May NOT be best choice when:
- Your pain is primarily from joints, not muscles
- You have contraindications to injections
- Trigger points are not identified on examination
Why Choose Palm Beach Regenerative for TPI?
- Fellowship-trained physician
- Can be enhanced with orthobiologic preparations
- Often combined with other treatments same visit
- Comprehensive approach to pain
- Systematic follow-up
- Part of complete treatment plan
Cost and Insurance
Many insurance plans cover trigger point injections. We provide:
- Insurance verification before treatment
- Transparent cost information
- Superbills for out-of-network plans
- Affordable cash pricing if needed
Required Disclaimer
Like all medical procedures, trigger point injections have potential risks and benefits. Results vary by individual. Not all patients achieve complete pain relief. Dr. Ghattas will discuss realistic expectations during consultation.

Hyaluronic Acid Injections
Hyaluronic Acid (HA) Injections
Hyaluronic acid (HA) is a naturally occurring molecule found in healthy joint fluid (synovial fluid) and cartilage. In an arthritic joint, HA concentration and molecular weight decrease significantly — reducing the joint’s natural lubrication, shock absorption, and biological signaling capacity. HA injections restore the viscosity and cushioning of the joint environment, reducing friction between articular surfaces and providing a medium more conducive to joint health.
At Palm Beach Regenerative, hyaluronic acid is not used as a standalone injection in most cases. It is frequently combined with PRP or BMC in the same procedure — an approach supported by growing clinical evidence showing superior outcomes compared to either treatment alone. The HA provides immediate lubrication and a favorable joint environment, while the PRP or BMC adds the biological activity that supports your body’s natural processes over time. All HA injections are performed under real-time image guidance to confirm accurate intra-articular placement.
| Treatment | Primary mechanism | Onset | Duration |
|---|---|---|---|
| HA alone | Lubrication and viscosupplementation | 1–4 weeks | 3–6 months typically |
| PRP alone | Biological activity — growth factors and cell signaling | 2–6 weeks | 6–12+ months variable |
| HA + PRP combined (PBRSS approach) | Lubrication + biological activity simultaneously | 1–4 weeks (HA) + delayed biological benefit | Superior and more durable than either alone in many patients |
| HA + BMC combined | Lubrication + broader cell and growth factor profile | Variable | Used for more advanced joint degeneration; selected cases |
Common applications at Palm Beach Regenerative
| Joint / Condition | Application note |
|---|---|
| Knee osteoarthritis | HA + PRP combination — frequently used; strong clinical evidence for combined approach |
| Hip osteoarthritis | Image-guided HA + PRP for carefully selected candidates (see Hip OA candidacy criteria) |
| Shoulder osteoarthritis | Glenohumeral joint HA + PRP for mild-moderate arthritis |
| Ankle and toe joints | HA + PRP for small mechanically loaded joints — particularly appropriate |
| Temporomandibular joint (TMJ) | HA viscosupplementation for TMJ arthritis and disc disorders |
Key points to remember:
- Image-guided — ultrasound or fluoroscopy confirms accurate intra-articular placement
- At PBRSS, HA is most commonly combined with PRP or BMC — not used in isolation
- The combination approach leverages lubrication (HA) + biological activity (PRP/BMC) simultaneously
- HA brand/formulation selected based on joint size, viscosity requirements, and treatment protocol
- Results vary; not all arthritic joints respond equally; candidacy depends on arthritis severity
- Does not regenerate cartilage — reduces friction, improves joint environment, supports natural processes

Stellate Ganglion Block
What is Stellate Ganglion Block?
Stellate Ganglion Block (SGB) is a specialized interventional procedure that targets the sympathetic nervous system to help manage complex pain conditions, especially Complex Regional Pain Syndrome (CRPS) and certain nerve-related disorders.
The stellate ganglion is a collection of nerve cells in the neck that is part of the sympathetic nervous system – the system that controls fight-or-flight responses. When this system becomes overactive after injury or surgery, it can cause severe, burning pain and other symptoms.
At Palm Beach Regenerative, Dr. Ghattas performs this advanced procedure with precision image guidance for optimal safety and accuracy.
How SGB Works
The Sympathetic Nervous System:
The sympathetic nervous system normally helps regulate blood flow, temperature, and other automatic body functions. After certain injuries or surgeries, this system can malfunction, causing:
- Severe burning pain out of proportion to injury
- Skin temperature and color changes
- Excessive sweating
- Hypersensitivity to touch
- Limited range of motion
The Procedure:
SGB involves precise injection of anesthetic medication near the stellate ganglion (located in the neck) to temporarily block abnormal sympathetic nerve activity. This may:
- Interrupt pain signals
- Help reset sympathetic tone
- Improve blood flow to affected limb
- Reduce inflammation
- Break pain cycle
Image Guidance:
Due to the stellate ganglion’s location near critical structures (arteries, veins, nerves, lung apex), Dr. Ghattas uses real-time fluoroscopic (X-ray) guidance or ultrasound visualization with contrast dye to confirm accurate placement.
Conditions Treated with SGB
Complex Regional Pain Syndrome (CRPS):
- Type I (no confirmed nerve injury)
- Type II (confirmed nerve lesion)
- Upper extremity CRPS
- Refractory to other treatments
Nerve Pain Conditions:
- Neuropathic pain in arm or hand
- Post-herpetic neuralgia (shingles pain) in upper body
- Phantom limb pain
- Post-traumatic neuralgia
Vascular Conditions:
- Raynaud’s phenomenon affecting hands
- Certain upper extremity vascular insufficiency conditions
Other Applications:
- Severe upper extremity pain not responding to other treatments
- Hyperhidrosis (excessive sweating) of hands
What Makes SGB Different?
Targets Root Cause:
Unlike treatments that mask pain, SGB may address underlying sympathetic dysfunction contributing to pain.
Diagnostic and Therapeutic:
Response to SGB helps confirm sympathetically-mediated pain and predicts response to further treatment.
Series Approach:
Often requires series of blocks (typically 2-10) for optimal and lasting results.
Specialized Expertise:
Requires advanced training and image guidance for safe, effective performance. Dr. Ghattas’s fellowship training in interventional pain management includes extensive SGB experience.
What to Expect During SGB
Pre-Procedure:
- Detailed history and physical examination
- Review of imaging and diagnostic studies
- Discussion of risks, benefits, and expectations
- Consent process
- IV access may be placed for safety
Procedure (20-40 minutes):
- Patient positioned on fluoroscopy or ultrasound table
- Monitoring equipment applied (blood pressure, pulse, oxygen)
- Neck area cleaned and sterile field created
- Image guidance used to identify stellate ganglion location
- Local anesthetic applied to skin
- Precise needle advancement under continuous imaging
- Contrast dye confirms proper position
- Anesthetic medication injected (typically 5-10cc)

Prolotherapy
What is Prolotherapy?
Prolotherapy is a time-tested regenerative treatment that stimulates your body’s natural healing response in ligaments and tendons. Using carefully formulated dextrose-based solutions, prolotherapy creates a controlled healing stimulus that may help strengthen lax ligaments and improve joint stability.
At Palm Beach Regenerative, Dr. Ghattas often combines prolotherapy with advanced interventional orthobiologics for comprehensive care of joint instability and chronic ligament conditions.
The term “prolotherapy” comes from “proliferant therapy” – stimulating the proliferation of cells involved in tissue repair.
How Prolotherapy Works
- Preparation
Dextrose-based solution (typically 12.5-25% dextrose) is prepared. Local anesthetic may be mixed with the solution for comfort. - Precision Injection
Using palpation or ultrasound guidance, Dr. Ghattas injects the solution into weakened ligaments, tendons, or around joints. This solution creates a mild, controlled inflammatory response that:- Stimulates your body’s healing cascade
- Promotes collagen production by fibroblasts
- Strengthens tissue with new structural proteins
- Improves joint stability through tighter ligaments
- Series Approach
Unlike cortisone (which suppresses inflammation), prolotherapy intentionally creates inflammation to jumpstart healing. Typically requires multiple treatments spaced 2-4 weeks apart for optimal results.
Conditions Treated with Prolotherapy
Joint Instability:
- Sacroiliac joint instability
- Chronic ankle sprains/laxity
- Knee instability from ligament laxity
- Shoulder instability
- Finger/thumb joint laxity
Ligament Conditions:
- Partial ligament tears
- Chronic ligament sprains
- Ligament laxity from overuse
- Post-injury ligament weakness
Tendon/Insertion Issues:
- Chronic tendinopathy
- Tennis/golfer’s elbow
- Plantar fasciitis
- Achilles tendinopathy
Chronic Pain Conditions:
- Sacroiliac joint dysfunction
- Chronic low back pain from ligament laxity
- Chronic neck pain from whiplash
- Headaches from cervical instability
What Makes Prolotherapy Different?
Mechanism of Action:
Unlike orthobiologics (which deliver cells or growth factors), prolotherapy works by creating controlled inflammation that stimulates your body’s own repair mechanisms.
Cost-Effective:
Prolotherapy is generally less expensive than advanced interventional orthobiologics, making it accessible for conditions that may respond to this approach.
Series Approach:
Typically requires multiple treatments (4-8 sessions) spaced 2-4 weeks apart for optimal results. Progressive improvement builds over the series.
Combination Potential:
Often combined with orthobiologics:
- Prolotherapy for ligament tightening
- PRP/BMC for cartilage or tendon support
- Comprehensive approach for complex conditions
What to Expect During Prolotherapy
Pre-Procedure:
- Evaluation of joint instability or ligament laxity
- Physical examination to identify specific structures
- Discussion of treatment series and timeline
Treatment (30-45 minutes):
- Treatment area cleansed
- Multiple small injections to ligament and tendon attachments
- May use ultrasound guidance for specific structures
- Local anesthetic may be mixed with prolotherapy solution
Total Time: Approximately 1 hour per session
Recovery and Timeline
Immediate (Day 0-3):
- Post-treatment soreness is expected and part of healing
- Mild swelling at injection sites
- Ice and rest as directed
- Avoid anti-inflammatory medications (they suppress desired response)
Between Treatments (Week 2-4):
- Soreness resolves
- Gradual strengthening of treated structures
- Continue normal activities as tolerated
- Next treatment scheduled
Treatment Series (2-6 months):
- Typically 4-8 treatments total
- Spaced 2-4 weeks apart
- Progressive improvement over series
- Final assessment 6-8 weeks after last treatment
Long-Term:
- Maintained improvement in many cases
- Lifestyle modifications may be needed
- Occasional maintenance treatments for some patients
Success Rates and Outcomes
Many patients with ligament laxity or joint instability report:
- Improved joint stability
- Reduced pain from instability
- Enhanced function and confidence in joint
- Decreased frequency of “giving way” episodes
Important Factors:
- Severity of instability
- Chronicity of condition
- Number of treatments completed
- Adherence to rehabilitation
- Underlying joint health
Realistic Expectations:
- Prolotherapy supports healing but cannot reverse advanced arthritis
- Some conditions require surgical stabilization
- Results build progressively over treatment series
- Not all patients achieve complete symptom resolution
Is Prolotherapy Right for You?
May be appropriate when:
- You have joint instability or ligament laxity
- Conservative care (bracing, therapy) hasn’t provided lasting improvement
- You have chronic tendon or ligament conditions
- You want to strengthen supporting structures around joints
- Cost is a factor and you have condition that may respond
May be combined with:
- Interventional orthobiologics (PRP, BMC) for comprehensive approach
- Physical therapy for strengthening
- Corrective exercises and ergonomic modifications
May NOT be best choice when:
- You have complete ligament rupture (may require surgical repair)
- You have severe arthritis as primary problem
- You cannot commit to treatment series
- You need immediate pain relief
Why Choose Palm Beach Regenerative for Prolotherapy?
- Evidence-based protocols
- Can be enhanced with ultrasound guidance
- Often combined with orthobiologics for comprehensive care
- Fellowship-trained physician
- Part of complete treatment approach
- Systematic outcome tracking
Cost and Insurance
Most insurance plans do not cover prolotherapy. We provide:
- Affordable option compared to advanced biologics
- Transparent pricing
- Superbills for possible reimbursement
- HSA/FSA eligible
- Treatment series pricing available
Required Disclaimer
Like all medical procedures, prolotherapy has potential risks and benefits. Results vary by individual. Multiple treatments are typically required. Not all patients are candidates. Dr. Ghattas will discuss realistic expectations during consultation.
